Pre Participation Physical

• CLICK HERE to access the Immunization and Health History form. 
• Complete the Immunization and Health History form, including the last 3 pages of athletic specific information, and have it signed by a PHYSICIAN (MD or DO).
• Submit to the Student Health Services Patient Portal, PyraMED, which can via accessed through JWULink.

SportsWare OnLine (Patient Management Software)

• Register on SportsWare
• Registration nstructions can be found HERE.
• Complete "My Info" Section (General, Address, Emergency, Insurance, Medical Alerts)
• Complete all forms designated as JWU-CLT

NCAA Athlete Sickle Cell Requirement

• Per NCAA policy, student-athletes must provide sickle cell blood test results. There are a variety of ways to obtain these results:
  • Student-athletes may obtain a sickle cell blood test
  • Student-athletes can obtain their newborn sickle cell blood test results

• Please upload your test results to SportsWare OnLine
  • Visit SportsWare OnLine
  • Go to “Forms” on left side of page
  • Click “Add” and upload results

NCAA Education

• 3 courses are to be completed annually. 1 of the courses is dependent on your year of athletic participation
• Instructions on accessing the courses can be found HERE
• Submit completion certificates via form linked in instructions

Annual Videos

• All athletes must watch the following video containing pertinent information about insurance prior to any athletic participation (CLICK ON THE IMAGE BELOW)
• Recommended to watch with parent/guardian who understands insurance
