ALL TASKS TO BE COMPLETED BY AUGUST 1ST

Pre Participation Physical

• CLICK HERE to access the Immunization and Health History form. 
  • Pages 1-3: University Health Services Requirements
  • Pages 4-6: Athletics Specific Requirements
• Complete the Immunization and Health History form, including the last 3 pages of athletic specific information, and have it signed by a MEDICAL PROVIDER
• Submit all six pages to the Student Health Services Patient Portal, PyraMED, which can via accessed through JWULink at JWULink>Tools>Health Services

SportsWare OnLine (Patient Management Software)

• Register for a SportsWare account.
• Registration instructions can be found HERE.
• Wait for Admin Approval. Approvals are made M-R, 9am-4pm.
• Complete "My Info" Section (General, Address, Emergency, Insurance, Medical Alerts)
• Complete the following sections: (CLICK HERE for detailed instructions)
  • Athlete Info
  • Forms
  • PHQ-9
  • Medical History

NCAA Athlete Sickle Cell Requirement

• Per NCAA policy, student-athletes must provide sickle cell blood test results. There are a variety of ways to obtain these results:
  • Student-athletes may obtain a sickle cell blood test
  • Student-athletes can obtain their newborn sickle cell blood test results
    • Each state has a database. You can find the contact for your state here: State Contact List (hyperlinked) https://newbornscreening.hrsa.gov/your-state

• Please upload your test results to SportsWare OnLine
  • Visit SportsWare OnLine
  • Go to “Forms” on left side of page
  • Click “Add” and upload results

NCAA Education

• The NCAA requires student-athletes to complete education on a variety of topics prior to participation. These courses will be assigned to you AFTER deposit has been made and a notification will be sent to your SCHOOL EMAIL
  • Search for SUBJECT LINE:  “JWU Required Online Course”
  • There will be 2 sections of courses, you will need to complete ALL courses
    • New To JWU Required Fall (2 courses)
    • Athlete (3 Courses)
  • These courses will take approximately 3-5 hours to complete.

Annual Videos

• All athletes must watch the following video containing pertinent information about insurance prior to any athletic participation (CLICK ON THE IMAGE BELOW)
• Recommended to watch with parent/guardian who understands insurance
