  • Forms (Annual Release of Information, Returning Athlete Assumption of Risk)
  • Medical History

NCAA Education

• NCAA requires student-athletes to complete education on a variety of topics prior to participation. These courses will be assigned to you AFTER deposit has been made and a notification will be sent to your SCHOOL EMAIL, sometime after June 1^st.
  • Search for SUBJECT LINE: “JWU Required Online Course”
  • These courses will take approximately 2-3 hours to complete.

Review NCAA Banned Substances and Required Forms; Complete If Applicable

• Review NCAA Banned Substances Information to determine if forms need to be completed.

  • NCAA Medical Exception ADHD Form
  • NCAA Medical Exception Pre-Approval Form???????
